Our Commitment

Wellness Experts is required by law to maintain the privacy of your protected health information (PHI), provide you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

How We May Use and Disclose Your PHI

Treatment: to coordinate and provide care, including sharing information with clinicians, pharmacies, and laboratories involved in your care.

Payment: to bill and obtain payment for services, including verifying coverage and eligibility.

Health care operations: for quality assessment, staff training, care coordination, and business management.

As required by law: for public health activities, health oversight, judicial proceedings, law enforcement requests, and to avert a serious threat to health or safety.

Uses Requiring Your Authorization

Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time.

Your Rights

Access: you may inspect and obtain a copy of your health record, generally within 30 days of a written request.

Amendment: you may request a correction to information you believe is inaccurate or incomplete.

Accounting of disclosures: you may request a list of certain disclosures we have made.

Restrictions: you may request limits on how we use or disclose your PHI, including asking us not to share information with your health plan for services you pay for in full out of pocket.

Confidential communications: you may request that we contact you at a specific address or phone number.

Paper copy: you may request a paper copy of this notice at any time.

Breach notification: you will be notified if a breach compromises the privacy or security of your PHI.

Electronic Communication

Email, text, and website chat are not fully secure channels. Please avoid sending detailed health information through them; we will use secure methods for clinical communication.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for information we already hold as well as any information we receive in the future. The current notice will always be posted on this page.
